Your GP will take a detailed sexual history to determine why your erections are failing and under what circumstances you are having sexual difficulties. You will also be asked about lifestyle factors (e.g. your job, work pressures, smoking habits, exercise, diet, alcohol intake and drug consumption). In some cases, it may be necessary to do specific diagnostic tests other tests. These may include: A nocturnal penile tumescence and rigidity (NPTR) test An intracavernous injection test Penile Doppler Ultrasound Neurological tests
Certain street drugs such as "poppers" also can cause serious problems if taken with PDE5i medications. These poppers are often types of nitrates and can cause severe drops in blood pressure. Ecstasy is another street drug that may increase sexual desire but interferes with performance. This has prompted some men to combine ecstasy with PDE5i medications. This mixture (a combination sometimes called "sextasy") can improve erection ability but also causes severe headache and priapism. (Priapism is an abnormally prolonged erection that becomes extremely painful and may result in permanent damage to the erection mechanism.) There are also potentially dangerous effects to your heart from mixing PDE5i medications with various other street drugs.

To diagnose ED, the doctor will ask about your symptoms and medical history. They’ll do a complete physical exam to look for signs like poor circulation or nerve trouble. They’ll also check for problems in your genital area that could cause trouble with erections.
When the aforementioned strategies prove insufficient to address ED symptoms, medical therapies are warranted. The simplest treatments include use of a class of oral medications known as phosphodiesterase type 5 inhibitors (PDE5I, e.g. sildenafil, tadalafil, vardenafil, and avanafil). These drugs enhance the effects of nitric oxide, a natural molecule within the body that relaxes the smooth muscle of the penis, to enable increased blood to flow and result in erection. These medications are generally very safe and reliable. Men who use nitrates (found in certain heart medications and in some recreational drugs) should not use PDE5I. Optimal use of PDE5I requires advice on timing and proper use of these drugs for maximal effect.

Infection is a concern after placement of a prosthesis and is a reported complication in 8%-20% of men undergoing placement of a penile prosthesis. If a prosthesis becomes infected (redness, pain, and swelling of the penis and sometimes purulent drainage are signs of infection), the prosthesis must be removed. Depending on the timing and severity of the infection and your surgeon's preference, the area can be irrigated extensively with antibiotic solutions and a new prosthesis placed at the same time or removal of the infected prosthesis and an attempt to place a new prosthesis made at a later time when the infection is totally cleared.
