The doctor will perform a complete physical examination, to check for abnormalities in the penis, scrotum, and testicles. To check for heart disease the doctor will take your blood pressure, measure your heart rate, and order a blood test to check your level of cholesterol. Your doctor will do a digital rectal examination to feel the size, shape, and consistency of the prostate, and if necessary check the level of prostate-specific antigen (PSA) in your blood. To check for diabetes the doctor may order a test to measure the level of glucose in your blood. It could also be necessary to assess your testosterone levels in the morning. This is also done with a blood test.
Second line therapies for ED include the use of intraurethral prostaglandin E1 (Muse), the vacuum device, and intracavernous injection therapies. .

Penile implant surgery can be very effective, provided that precautions are taken to avoid infection. Prosthesis surgery is contraindicated if systemic cutaneous or urinary infection is present. Antibiotics should be provided pre-operatively, and the surgical site should be shaved immediately prior to surgery. We use both Mentor and AMS penile implants with specialized antibiotic coats. Patients are hospitalized overnight for closed suction drainage, and sent home on seven days of post-operative antibiotics. Using these and other precautions, our implant infection rate is comparable to national averages (2-4%, 1-2% for antibiotic coated implants). Vascular surgery is recommended only in healthy individuals with recently acquired erectile dysfunction due to a focal arterial narrowing (usually related to trauma) and in the absence of generalized vascular disease. A malleable penile prosthesis usually consists of paired rods that are inserted surgically into each of the corpora cavernosa. The rods are stiff, and basically to have an erection, one bends them up and when finished with intercourse they are bent down. They do not change in length or width. The malleable penile prosthesis has the lowest risk of malfunction, however they have the least normal appearance.

Asian ginseng and ginkgo biloba (seen here) are popular, but there isn't a lot of good research on their effectiveness. Some men find that taking a DHEA supplement improves their ability to have an erection. Unfortunately, the long-term safety of DHEA supplements is unknown. Most doctors do not recommend using it.
